Sorry, this is originally a WordPerfect® file. I have created an RTF that you can edit using Microsoft® Word. The RTF is OK but more wasteful of paper. I haven't got around to doing it properly in Word, yet. The PDF is just so you can see what it should really look like if you don't have WordPerfect®.
Edit the sheet to suit your organisation. Note that the repeated information is in the page header and that it changes half way through the document.
We make two copies and staple them together, using one staple per slip. We cut the top copy into strips if there are several per A4 page, so that each one is just held by the staple. Choir members sign up for tasks by tearing off the slips and writing their names on the second copies. This way the committee knows who has agreed to do what.
